What do I do with this file?

As it says on the download page you get to when following the link:

extract to your \Everquest II directory

If you keep having trouble with unzipping, you may want to give the auto-updating version of ProfitUI a try (link in my sig).

If it does not work, you likely installed it incorrectly. Always try a clean install before reporting bugs.
