Quest Journal Oddities

Anyone else seeing the "completed", "heroic" and "epic" tags in quests in the main quest journal dissappearing and reappearing?

I have not figured out what I am doing to cuase them to appear and disappear, but they are coming and going.

this sounds like a game /bug

I figured it might be a game bug, but wanted to confirm that others were seeing it before I did a /bug report in game. (I am getting old, and thought maybe i was just losing my mind)
