you could try zooming out into third person and then ( if you haven't remaped them ) use the / and * on the numberpad of your keyboard to move the camera to where it is facing you, this would let you have some idea of what might be chasing you

an actual rear view mirror? no. Not without being able to edit source code to EQ2 of course.

Default Looking behind you ...

I know the issue he is talking about and the trick of looking back by going 3rd person and spinning the camera doesn't work while swimming. But prior comment may be something to check - is there a way to shift the paperdoll view to see back there?

Default sorta similar

what I did with my own UI is close to this - I play a monk, and wanted to be able to see my combat moves while fighting without changing the view - so I moved the paperdoll to an "always up" window and resized it very small (something like 1 inch x 1.5 inches) ,

I use it like a "rear view" sorta - while it wont show whats behind me, it does show me when I'm getting hit - so I know when I've outrun something chasing me for example... its also very useful when playing with the sound off - thats as close as I've gotten to what you describe.

Something similar is in place in the game Guild Wars. I believe hitting the X key by default switches your view as if you were standing in front of yourself, looking at yourself and what's behind you, for as long as the key was depressed. You could be autorunning away from bad guys, press X and see what was behind you. You could even maneuver your character around while looking behind if you got the hang of reversing the movement keys. The view was equidistant to how far you had the camera in third person. I really liked this feature, and it made checking the status on things chasing you a breeze.
