All I know is that in order to get alive and back to where ever you consider safe, i have to load the default UI and then get some wheres safe and log and come back in with Fetish and continue to play the game.

Not sure who is at fault but it is very very annoying.

Read above. Since SOE changed the way death works, it causes Fetish (and maybe other UIs, dont know) to crash upon death. Rename the eq2_mainhud_death.xml to something like eq2_mainhud_death.xml.BAK, and it'll then load the default revive window - should fix the crash problems until Zonx has an update.
