I tested this with nothing but EQ2MAP installed Othesus. On my system it lines up perfectly like that (actually the menu could probably be moved 1 pixel to the left to be aligned with the icon).

Alignment differences between systems/servers?

If it does not work, you likely installed it incorrectly. Always try a clean install before reporting bugs.
