the screenshot shows the target casting ice comet so that is covered. the idea of having the click buttons for the right 2 groups closer to the center sounds good though.
The only other thing i can think of would be to have one of your hotkey bars (maybe hotbar 9 since 10 is used for the spelltimer window) replicated in the gap under the self cures. Either that or add another row of click buttons that you can set up to cast the spells you want.
