Fetish should not affect your voice chat -- make sure you've enabled it via options.
As for the POIs... install Fetish, then download, install, and run the EQ2MAP updater and all your POIs should be there. When using the updater, if it doesn't say its installed to FetishNightfall (but instead says EQ2MAP) you have things set up wrong.
