This may not be the problem for everyone, but if you're running a localized version other than the one that came out of the box, you need to place eq2.ini in the localized version's base folder, and the mod folder in THAT UI folder.

Example: I have the US version and the German localization. The game's base folder is E:\EQ2.

To run Insom when playing the English version, I place
* eq2.ini in E:\EQ2
* eq2ui_skininfo.xml (and the rest of it) in E:\EQ2\UI\Insom

When playing the German version, I need to place
* eq2.ini in E:\EQ2\LP_REGION_DE_DE
* eq2ui_skininfo.xml (and the rest of it) in E:\EQ2\LP_REGION_DE_DE\UI\Insom

Hopefully that nails down everything. Just make sure the files are unzipped to the folder from which Everquest2.exe (the game itself, not the launcher) is being run.
