Look in your custom UI directory and if you do not see a file called eq2ui_custom.xml.
If the file does not exists or you do not have a custom UI directory then you can extract all the file from the zip into a custom UI directory.
If the file does exists then you need to add the following lines just above the line that says </Page>.

Once in game you have to issue the command /show_window Custom.StatBar.
