Vista has known problems in this area. The main one is that by default EQ2 gets installed into the Program Files tree. Vista by default prevents alteration of files in that tree, which means the changes to eq2.ini to enable custom UIs can't happen. You'll need to mess with Vista's security settings to disable UAC, and possibly run as administrator.

My recommendation: switch back to XP. Probably not feasible, but things like your experience convince me that Vista's just not suitable as an OS for people who aren't professional systems administrators.
