Try the following:

Start EQII, using the Fetish UI.

Open up Options->Controls->Window Keys. Look for the 'Maintained Spells' line. If there is no key assigned, assign it one (any will do).

You should be able to use that key to make the window appear (it is a toggle). Once you have it, r-click and select 'Window Settings'. Press the 'Default Location' and then the 'OK' button.

That should have 'normalized' that window. Log off and log back on, and see if the window is present and in the same location you left it. If so, you should be able to move it around now to where you want it to be.

I am thinking that either the window is toggled to 'off' when you load Fetish, or it is being displayed outside the boundaries of your display window, or it has been resized down to something like one-pixel in the x or y direction (something I do by accident now and then).
